Provides assessment, treatment and therapy for adults with a learning disability to increase personal independence and choice and promote better health and quality of life
How it can help
The Learning Disability Service supports adults to access other primary and secondary care services, including health facilitation and ensuring that reasonable adjustments are in place.
They also deliver direct support for people and their families. This includes:
- assessment and formulation of needs to inform personalised care and support plans
- evidenced based interventions including a variety of treatments and therapy
- support to service providers and families who care for people with a learning disability who have complex and continuing health needs.
- emergency mental health support
The support the Learning Disability Service can provide includes:
- physical health
- daily living skills
- eating and drinking
- medications
- behaviour, thoughts and feelings
- memory difficulties
- communication
- comfort, breathing, and how you move

Eligibility
Adults with a learning disability who are registered with an Essex GP and whose needs cannot be met by mainstream services
